Ageing and Inclusion

Corangamite Shire Council has concluded providing Assessment and In Home Aged Care services on behalf of the Commonwealth and State Governments from June 30 2023.

Aged Care and Assessment services are now provided by a range of new provider organisations across the Shire.

Navigation Support Program

Council has commenced a new program called Navigation Support Program to assist seniors and vulnerable people access the services they need.  We are here to help:

  • Assist residents in understanding the Aged Care system including eligibility, access, assessment and how to find a service provider.
  • Supporting residents to link in with other agencies including social services, health and community participation opportunities.
  • Assist residents to become involved in active aging programs.
  • Arranging activities to support residents be healthy, active and connected.
  • Assistance with food relief.
  • Linking Carers to support services and networks
  • Community advocacy
  • Delivery of Council’s Disability Access & Inclusion Plan 2021-2025

The Navigation Support Program is a free and confidential service.

Charter of Aged Care Rights

You have a right to receive high quality care and services and to always be treated with respect and dignity. 

These rights and more are protected by the Charter of Aged Care Rights. The Charter provides the same rights to all consumers. These rights apply no matter what type of Australian Government funded aged care services you receive. 

It is a good idea for you and those involved in your care to familiarise yourself with your rights. All providers of Australian Government funded aged care must comply with the Charter.
